I affirm, under penalty of perjury and as a condition of purchase, that:

1. I am at least 21 years of age, or the age of majority in my jurisdiction, whichever is greater.
2. I have the legal capacity and authority to enter into this agreement, and if I am purchasing on behalf of an entity, I am authorized to bind that entity.
3. I am acquiring these materials solely for lawful laboratory, analytical, or educational research purposes.
4. I will not use, administer, or permit the use of these materials in or on the human body, by any route, including injection, ingestion, inhalation, or topical application.
5. I will not administer these materials to any animal and will not use them for any veterinary purpose.
6. I will not resell, transfer, distribute, or dispense these materials to any person for human consumption or administration, and any resale will be solely to qualified research purchasers under materially the same terms as these.
7. I am qualified and equipped to handle, store, transport, and dispose of research chemicals safely, and I accept sole responsibility for doing so.
8. I understand these materials are not drugs, dietary supplements, cosmetics, or medical devices, are not approved by FDA or any other authority for any use, and have not been evaluated for safety or efficacy in humans or animals.
9. I will comply with all applicable federal, state, local, and international laws and regulations, including import, export, and controlled substance requirements.
10. Neither I nor my organization is a sanctioned or denied party or located in a sanctioned jurisdiction, and I will not export or re-export these materials in violation of trade controls.
11. I will notify [COMPANY LEGAL NAME] in writing before any change in my intended use, and I understand no change in intended use is authorized without written consent.
12. All information I have provided is true and accurate, and I understand [COMPANY LEGAL NAME] may verify it and may refuse, delay, or cancel any order.
